Remediation timing, triage timing, reopen rate, and open aging

Time-to-remediate / time-to-triage / reopen-rate per severity + open aging.

A pure fold over the append-only status_history — no new data capture. Mean/median/p90 are computed over one duration per finding (the aggregation never hydrates documents). Legacy genesis-less rows clock-start from discovered_at.

• Scope required: findings:read.
• Response supports ETag revalidation (304 Not Modified).
